Eligibility: This course is open to the 4 Wing Defence Team, including military and civilian personnel, and family members aged 18 years and older.

Registration: Registration is required to attend the course. Military and civilian personnel require supervisor approval.

• Written proof of approval from Supervisor/CoC; and
• SN, Full Name, Rank, Local and Unit of the member/employee
• Complete the online form at: http://www.cafconnection.ca/Cold-Lake/Adults/Health/Health-Promotion/Registration

*Note that military personnel have priority. Civilian participants may be removed from the course should registrations exceed the maximum number permitted. If the course is full when your registration is received, you will be notified that you have been placed on the waitlist.
